$50 and hour for design / revision work. If I have all the images you want in the correct format this should only take about an hour. If I need to redraw, edit, or find images this time will go up considerably. The plate charge and set up fee is $150. There is a minimum of 50 sheets at $5 a sheet so for your project at minimum it will be $450 and we are currently at a 3 month ETA. Any additional design work will be billed separate.
